Cigna Contribution Scheme:

  • 75% for employee 65% for dependents for the Cigna OAP Base and Buy Up Plans
  • 80% employee and 75% for dependents for the Cigna HDHP

 

Kaiser Contribution Scheme:

  • 75% of base plan for employee
  • 65% of base plan for each of the dependents

Principal Dental Contribution Scheme:

  • 75% for employee
  • 60% for each of the dependents

Principal Vision Contribution Scheme: 

  • 0% for Employee
  • No contribution for Dependents 

Exams:

  • Every 12 months, one exam is covered in full after $10 co-pay

Flexible Spending Account (FSA) - WEX

  • Pre-Tax deduction to help pay for eligible medical, dental, and vision expenses
  • Account is 'owned' by Kognitiv and updates to HCFSA can only be made at OE or
    through a life event
  • $3050 Limit for Health Care FSA, $5000 for Dependent Care FSA
  • "Use it or lose it" for all funds added to FSA account during the plan year (with 75 day
    grace period)

Health Savings Account (HSA) - WEX

  • Must be enrolled in Kognitiv's High Deductible Health Care Plan to qualify
  • Pre-Tax deduction to help pay for eligible medical, dental, and vision expenses
  • Account is 'owned' by employee and contribution amounts can be changed at any time
  • $3850 Individual/$7750 Family Limit
  • Funds in a HSA roll over and never expire

Voluntary Life (Employee, Spouse, Children) - Principal

  • Employee Paid Post-Tax
  • Employee Life: Increments of $10,000 up to $500,000, Guarantee Issue of $150,000
  • Spouse Life: Increments of $10,000 up to $250,000, Guarantee Issue of $30,000 -
    must not exceed Employee Life election, premium based on employee age
  • Child Life: $10,000 - child must be under the age of 26

We understand the importance of family, which is why we provide generous parental leave, ensuring our employees have the time and support they need to embrace the precious moments of parenthood.

  • 12 weeks paid leave
  • Leave can be spread out to fit the new parents schedule based on Manager Approval (example: every Monday for 60 weeks)
  • Employees are eligible to expense a six month SNOO bassinet rental

Employees will be reimbursed up to $500 for approved home office equipment.
